Abstract

The toolkit was developed from the findings of an Evaluation of the Social Care Role in Integrated Teams in Nottinghamshire. The evaluation found that placing social care workers in integrated teams was cost effective and made a valuable contribution to integrated working but only if certain conditions were in place. Evidence from the evaluation has been used to create a toolkit to help promote the conditions that enable successful integrated working. The toolkit has been designed to support managers, integrated teams and social care workers to deliver the social care role effectively in an integrated setting. If you are intending to use the toolkit could you please provide information about how you will use the toolkit, or give any feedback you have regarding the toolkit, using the online questionnaire accessible from the Official URL link.
